How Monarch Does an Independent Medical Evaluation

  • Step One: Request an Appointment

    Request a mental health assessment appointment here.

  • Step Two: Informed Consent & Payment

    You will receive an email from Monarch with paperwork to complete.  Complete and sign our informed consent form and make a payment. (This should come to your email)

  • Step Three: Required Documentation

    Your provider will send you documentation to complete. Gather all your required information and submit your documents through the client portal prior to your scheduled assessment

  • Step Four: Online Assessment

    Your provider will meet with you online to go over your paperwork, and administer your mental health assessment. 

  • Step Five: Provider Review

    Your provider will handle the rest, reviewing your documents and prepare a comprehensive nexus letter for you.

Veteran's Disability Compensation Table
